Explanation:
City of Hollywood is a member operator of the Underground Facility Damage Prevention and Safety Act legislated by the State of Florida under Chapter 556. This Act has created a uniform Statewide procedure for determining the existence of underground utilities prior to excavation. This system provides member operators an opportunity to identify and locate underground facilities to avoid potential accidents without having the need for excavators or excavating contractors to contact each and every underground utility provider (known as member operators under the Act) to determine whether such utilities are present in the area to be excavated.

The City's current blanket purchase order with Craig A. Smith for the subject services expired on April 11, 2021 and the Departments of Public Utilities and Development Services desire to issue the attached blanket purchase agreement for a one-year period.

On July 3, 2019, the City Commission passed and adopted Resolution No. R-2019-209 that approved and authorized the issuance of a Blanket Purchase Order to Craig A. Smith & Associates for underground utility location services for an initial one-year term with the option to renew for two additional one-year terms based upon the Town of Pembroke Park's RFP 05-1295 in accordance with Section 38.40(C)(5) of the City's Purchasing Ordinance (Piggyback).
