Explanation:
The Department of Human Resources requires an agreement with a qualified vendor to provide health screening services including a comprehensive medical physical/examination for eligible Fire, Police, General, Professional, Supervisory and Non-Represented personnel in order to provide the City's contractually negotiated annual medical examination benefit paid for by the City.

The City's most recent incumbent for health screening services is Life Extension Clinics, Inc. D/B/A Life Scan Wellness Centers, but the one-year contract expired May 31, 2019.

The City began the RFP process on May 15, 2019 when Request for Proposal number RFP 4615-19-PB was issued for an initial time period of one year and three optional one year renewal periods with a flat rate paid for by the City with health insurance reserves.

The RFP process was completed on June 3, 2019 and on July 3, 2019, the City Commission adopted Resolution No. R-2019-199 which inadvertently authorized an agreement for an initial three year period with the option to renew for two additional one year periods.

This resolution has been revised to request authorization of an Agreement for a one year initial period with the option to renew for three additional one year periods for health screening services, for an annual expenditure not to exceed 110% of $450,000.00.

Funding for this agreement has been provided in the Health Claims Reserves budget account number 558.112006.51900.59...
