Explanation:

The Department of Human Resources requires an agreement with a qualified vendor to provide health screening services including a comprehensive medical physical/examination for eligible Fire, Police, General, Professional, Supervisory and Non-Represented personnel in order to provide the City’s contractually negotiated annual medical examination benefit paid for by the City.

 

In addition, the Department of Human Resources will make available the health screening services including a comprehensive medical physical/examination to dependents covered under the City’s insurance, with all costs to be borne by the dependent and at no cost to the City.

 

The City’s most recent incumbent for health screening services is Life Extension Clinics, Inc. D/B/A Life Scan Wellness Centers, but the one-year contract expired May 31, 2019.

 

Therefore, on May 15, 2019, Request for Proposal number RFP 4615-19-PB was issued for a time period of three years with two optional one year renewal periods with a flat rate paid for by the City with health insurance reserves.

 

Notices were mailed and the RFP was advertised online via the City’s website and BidSync in accordance with the City’s Purchasing Ordinance, Section 38/42(A)(1), and resulted in the responses from two firms as follows:

 

Life Extension Clinics, Inc. d/b/a Life Scan Wellness Centers

 

Tropical Coast Medical Associates, Inc.

 

The evaluation of the submittals was based upon the criteria established in the RFP by a four member evaluation Committee consisting of the Director of Human Resources, Division Fire Chief, Fire Driver/Engineer and Police Major.

 

The Evaluation Committee reviewed the proposals and scored the firms based on the established RFP criteria and determined the scores to be as follows:

 

1.                     Life Extension Clinics: 346 points

2.                     Tropical Coast Medical Associates: 340 points

                

Based on these results, the Evaluation Committee recommends that the City Commission authorize Life Extension Clinics to be the vendor that provides the health screening services for the City.

 

The initial term of the Agreement will be three years with the option to renew for two additional one year periods for health screening services, for an annual expenditure not to exceed 110% of $450,000.00.

 

Funding for this agreement has been provided in the Health Claims Reserves budget account number 558.112006.51900.591990.000000.000.000 and will be budgeted in subsequent fiscal years.

 

Award is subject to the City’s receipt and approval of all insurance certificates required by the City’s Risk Manager, along with signed statements of Hold Harmless and Indemnity to the City.
