Explanation:
Memorial regional hospital was the first hospital in Broward County to perform adult heart transplant surgery. The cardiac and vascular institute uses a skilled team approach to provide quality heart care in the community focusing on prevention, treatment, and follow-up for a range of heart and vascular conditions.
During the implementation period from March 9, 2016 to October 31, 2018, approximately 1,200 patients were in the program - 400 of those patients (30%) were Hollywood residents. This program increased the rate of patients who were discharged directly home instead of to a nursing home or inpatient rehabilitation program from 42% to 86%.
