Explanation:

 

The Department of Human Resources required an agreement with a qualified vendor to provide health screening services, including a comprehensive medical physical/examination, for eligible Fire Rescue & Beach Safety personnel in order to provide the City’s contractually negotiated annual medical examination benefit paid for by the City.

 

In addition, the Department of Human Resources made available the health screening services, including a comprehensive medical physical/examination, to ancillary City personnel with all costs to be borne by the employee and at no cost to the City.

 

Health screening services, including comprehensive medical physical/examinations, for eligible Fire & Rescue Department Personnel/Ancillary Personnel was competitively solicited by the City of Jacksonville, Florida via the issuance, processing and award of RFP #P-41-13.

 

On August 25, 2014, the City of Jacksonville, Florida   awarded RFP #P-41-13 to Life Extension Clinic, Inc. d/b/a Life Scan Wellness Centers located in Tampa, Florida to provide health screening physicals for an initial contract period of six (6) months from October 1, 2014 through March 31, 2015  with the option to renew for three (3) additional one (1) year periods, under the mutually agreed upon terms and conditions and RFP #P-41-13.

 

On May 18, 2015, the City of Jacksonville, Florida renewed the agreement with Life Extension Clinic, Inc. d/b/a Life Scan Wellness Centers to provide health screening physicals for the contract’s initial one (1) year renewal period from April 1, 2015 through March 31, 2016 with the option to renew for two (2) additional one (1) year periods, under the mutually agreed upon terms and conditions and RFP #P-41-13.

 

On December 2, 2015, the City Commission passed and adopted Resolution Number R-2015-370, which authorized the issuance of blanket purchase order number B002810 to Life Extension Clinic, Inc. d/b/a Life Scan Wellness Centers to provide health screening services, including a comprehensive medical physical/examination, for eligible Fire Rescue & Beach Safety personnel in order to provide the City’s contractually negotiated annual medical examination benefit paid for by the City and to make available the health screening services, including a comprehensive medical physical/examination, to ancillary City personnel with all costs to be borne by the employee and at no cost to the City, in accordance with RFP #P-41-13, for an initial period expiring on March 31, 2016 with the option to renew for two (2) additional one (1) year periods for health screening services, for an estimated expenditure of $75,000.00.

 

On February 25, 2016, the City of Jacksonville, Florida renewed the agreement with Life Extension Clinic, Inc. d/b/a Life Scan Wellness Centers to provide health screening physicals for the contract’s second one (1) year renewal period from April 1, 2016 through March 31, 2017 with the option to renew for one (1) additional one (1) year period, under the mutually agreed upon terms and conditions and RFP #P-41-13.

                     

Life Extension Clinic, Inc. d/b/a Life Scan Wellness Centers  has supplied the required services to the City of Hollywood in a satisfactory manner and has agreed to renew their contract for the second one (1) year renewal period.

 

The Office of Human Resources has requested that thirteen (13) Fire Rescue & Beach Safety Management personnel be added to the eligibility list of City personnel to receive the health screening services, including a comprehensive medical physical/examination paid for by the City.

 

The addition of the thirteen (13) Fire Rescue & Beach Safety Management personnel will not increase the blanket purchase order number B002810 estimated annual expenditure of $75,000.00.

 

The Director of Human Resources and the Contract Compliance Officer have recommended that the City Commission approve the issuance of the attached blanket purchase order for the second one (1) year renewal period expiring on March 31, 2017, with the option to renew for the contract’s third/final one (1) year renewal period, for an estimated expenditure of $75,000.00.

 

Award is subject to the City’s receipt and approval of all insurance certificates required by the City’s Risk Manager, along with signed statements of Hold Harmless and Indemnity to the City.

 

Funding for this agreement has been provided in the FY16 Health Claims Reserves budget account number 58.1241.00000.590.009199 and will be budgeted in subsequent fiscal years.
