Explanation:

The Office of Human Resources requires an agreement with a qualified medical facility or physicians’ group licensed by the State of Florida to provide pre-placement physical examinations, complete physicals for members of the hazmat team, and to provide vaccinations, treatment and screenings of infectious diseases for the Fire Rescue and Beach Safety, Police, Public Utilities and Public Works Departments.

 

On March 20, 2015, Request for Proposals (RFP) Number 4453-15-RD was electronically advertised via BidSync in accordance with the City’s Purchasing Ordinance, Section 38.42(A).

                     

RFP-4453-15-RD for Occupational Health Services was opened at 3:00 P.M., on April 14, 2015 and resulted in five (5) responses as follows:

 

                                          Broward Outpatient Urgent Care, LLC

                                          Pompano Beach, Florida

 

                                          MD Now Medical Centers, Inc.

                                          West Palm Beach, Florida

 

                                          Robert Warren Enterprises d/b/a Occupational Medicine Center of America

                                          Miramar, Florida

 

                                               Solantic/South Florida, LLC d/b/a CareSpot Express Healthcare

                                          Pembroke Pines, Florida

 

 

                                          U.S. HealthWorks Medical Group of Florida, Inc.                                                               

                                          Fort Lauderdale, Florida                                                                                                                                                   

 

The RFP document’s selection process required an evaluation committee to review and score/rank the responsive proposals based upon the Evaluation Criteria established in the RFP document.

 

The responses were evaluated based upon the following criteria  1) Previous Experience and Past Performance, 2) Price  Proposal, 3) Project Understanding, Proposed Approach and Methodology, 4) Office Location based upon proximity to the City of Hollywood City Hall, 2600 Hollywood Blvd. Hollywood, Florida 33020 , 5) References and successfully completed similar projects.                     

 

The RFP document included a fee schedule for services required, to be completed by each proposer.

 

On May 14, 2015, the proposals were evaluated by a four (4) member committee representing the Office of Human Resources, Public Works, Fire Rescue & Beach Safety and Public Utilities.

 

The five (5) responsive proposals received were evaluated and the scores were compiled to arrive at the following rankings, (highest score), first choice to fifth choice:

 

1.                     MD Now Medical Centers, Inc.                                                                                    420

2.                     Occupational Medicine Centers of America                                          378

3.                     U.S. HealthWorks Medical Group of Florida, Inc.    373

4.                     Solantic/South Florida d/b/a CareSpot                                                               305

5.                     Broward Outpatient Urgent Care, LLC                                                               231

                                                                                

After evaluation of the responses, the Evaluation Committee recommends that the City Commission authorize the appropriate City staff to execute the attached Agreement and issue the attached blanket purchase order with the highest ranked firm, MD Now Medical Centers, Inc. for Occupational Health Services, in an estimated annual expenditure of $53,625.00.

 

The Agreement will be executed for a one (1) year period, with the option to renew for two (2) additional one (1) year periods.

 

Award is subject to the City’s receipt and approval of all insurance certificates required by the City’s Risk Manager, and indemnity to the City, along with signed Statement of Hold Harmless and Indemnity.

 

Funding for these agreements is available in the FY15 Account Numbers 01.2151.00000.522.003116 (Hazmat Physicals), 01.1132.00092.513.003116 (Pre-Employment Physicals), and 58.1241.00000.590.003117 (Contractual Services), and will be budgeted for subsequent fiscal years.
