Explanation:
The Department of Public Utilities requests approval of Change Order No. 14 with Cardinal Contractors, Inc. ("Cardinal") for a 150-day time extension for the Deep Injection Wells No. 3 and No. 4 Pump Station Project at the Southern Regional Wastewater Treatment Plant. This request does not include additional funding.
Brown and Caldwell, the Engineer of Record, has identified significant technical issues that directly affect the project's ability to be completed within the existing contract time. The newly installed automatic strainers have failed twice due to excessive solids loading, requiring additional engineering evaluation and corrective redesign to ensure long-term operability of the new pump station. In addition, the existing slide gate in the diversion box adjacent to Clarifier No. 4 has seized in the open position. Although a replacement slide gate has been ordered, delivery is not expected until July or August, which occurs after the current contract completion date. Installation of the new slide gate requires extensive dewatering operations and is essential to proper pump station functionality, making it impossible for Cardinal to perform the work before contract expiration.
