Explanation:
The City of Hollywood is certified by the State of Florida as a workers' compensation self-insurer and as such the City is subject to annual assessments by the Special Disability Trust Fund (SDTF) and the Workers' Compensation Administration Trust Fund (WCATF).  Each fiscal year the legislature specifies the rate of assessment for each of these funds based on various factors including payroll and loss experience.  The total assessment for the Fiscal Year 2014 was $56,022.95 which is paid in four installments with the first three having already been paid via Direct Payment Voucher.  The fourth installment is now due in the amount of $14,762.44 including fees.
