Explanation:
The Office of Human Resources & Risk Management and the Department of Fire Rescue & Beach Safety require an agreement for Occupational Health Services to provide pre-placement physical examinations, complete physicals for members of the hazmat team and to provide vaccinations, treatment and screenings of infectious diseases to certain employees.
On March 6, 2012 the Procurement Services Division advertised and issued RFP-4313-12-IS seeking a qualified vendor to provide the requested Occupational Health Services. The RFP responses were opened on March 29, 2012 which resulted in two (2) responses.
After evaluation of the responses, the evaluation committee recommended that the City Manager authorize the execution of an agreement with the highest ranked firm, U.S. HealthWorks Medical Group of Florida, Inc. for Occupational Health Services, for an estimated annual expenditure of $30,489.00.
On May 15, 2012 the City Manager via memo File:PR-12-144 approved the execution and issuance of blanket purchase order number B002397 to U.S. HealthWorks Medical Group of Florida, Inc. of Ft. Lauderdale, Florida to provide Occupational Health Services, in accordance with RFP-4313-12-IS for a one (1) year period with the right to renew for two (2) additional one (1) year periods, for an estimated annual expenditure of $30,489.00.
On April 1, 2013 the City Manager, via memo File:PR-13-102, approved the renewal of blanket purchase order number B002397 with U.S. HealthWorks Medical Group of Florida, Inc. of Ft. Lauderdale, Florida to provide Occupational Health Services, in accordance with RFP-4313-12-IS for a one (1) year period with the right to renew for one (1) additional one (1) year period, for an estimated annual expenditure of $40,000.00.
The requesting two (2) Departments have projected an annual operational need for an increased quantity of pre-placement physicals and related occupational health services to be required during the renewal period.
The estimated additional services have increased the estimated annual services expenditure to a dollar amount in excess of $50,000.00, which requires City Commission approval.
U.S. HealthWorks Medical Group of Florida, Inc. continues to provide Occupational Health Services to the City of Hollywood in a satisfactory manner.
U.S. HealthWorks Medical Group of Florida, Inc. has agreed to renew this contract for the final one (1) year renewal period under the terms and conditions of RFP-4313-12-IS.
The participating Departments and the Director of Procurement Services recommend that the City Commission approve the issuance of the attached Blanket Purchase Order renewing the agreement with U.S. HealthWorks Medical Group of Florida, Inc. of Ft. Lauderdale, Florida for the final one (1) year renewal period for an estimated annual expenditure of $54,500.00.
Renewal is subject to the City's receipt and approval of all insurance certificates required by the City's Risk Manager, and indemnity to the City.
Funding for the attached Blanket Purchase Order has been provided for in the fiscal year 2014 Office of Human Resources & Risk Management budget Account Numbers 01.1132.00092.513.003116 - Medical Services 58.1200.00000.242.740000 - Expenditures-Health Insurance and the Fire Rescue & Beach Safety budget Account Number 01.2150.00000.522.005201- Operating Supplies/Cost - Hazmat and funding for the costs anticipated to be incurred in FY 2015 will be requested as part of the FY 2015 proposed operating budget.
