Explanation:
The Department of Community and Economic Development requires a pool of qualified contractors to provide housing rehabilitation/repair services in conjunction with the City's Housing Rehabilitation Program, which is funded using Home Investment Partnership (HOME), State of Florida Housing Initiatives Partnership Program (SHIP) and the Department of Housing and Urban Development Community Development Block Grant (CDBG) funds in an estimated amount up to $600,000.00.
On July 3, 2014, Notices of Availability of Request for Qualifications (RFQ) Number 4420A-14-IS were mailed and legally advertised on the City's website and Demandstar.com in accordance with the City's Purchasing Ordinance, Section 38.42(A)(1).
RFQ-4420A-14-IS was opened at 3:00 p.m., on Thursday, August 7, 2014, which resulted in responses from the following Eleven (11) proposers:

The bid submitted from Construction Affairs, Inc was determined to be non-responsive as they failed to provide several documents, including the acknowledgement and signature page, noncollusion affidavit, acknowledgement of conformance with O.S.H.A. standards, sworn statement pursuant to section 287.133 (3) (a) Florida statutes on public entity crimes, certifications regarding debarment, suspension and other responsibility matters, drug-free workplace program, solicitation, giving and acceptance of gifts policy and request for taxpayer identification number and certification (W-9) as required in the bid document.
Evaluation of the proposals submitted for the Housing Rehabilitation Program was based upon the criteria as established in the RFQ as follows: general quality of submittals; company profile; professional competency; relevant work experience and financial competency.
On September 10, 2014, an Evaluation Committee consisting of the Department of Community and Economic Development Project Manager, Director of Building Department and Public Works Architect met to evaluate all eleven (11) proposals and a shortlist was created with five (5) contractors scoring 261 and higher as follows:
· Assured Contracting LLC
· Cosugas, LLC
· Daniel Secu Corporation (DSG Construction)
· Gentile Corp.
· Stacy Bomar Construction
The shortlist of contractors will be utilized for a two (2) years period for repair and rehabilitation of owner occupied dwellings on behalf of households that meet specific income guidelines.
The Director of Community and Economic Development and the Director of Procurement Services have recommended that the City Commission authorize the appropriate city officials to utilize the five (5) shortlisted contractors listed above to provide Housing Rehabilitation Services to participants in the City of Hollywood's Housing Rehabilitation Program utilizing Home Investment Partnership (HOME), State of Florida Housing Initiatives Partnership Program (SHIP) and the Department of Housing and Urban Development Community Development Block Grant (CDBG) funds.
