Explanation:
The 2008/2009 Wastewater System Master Plan Identified The Need To Rehabilitate The Existing Bar Screen Chamber At The Southern Regional Wastewater Treatment Plant.
On June 20, 2012, Via Resolution No. R-2012-188, The City Commission Approved The Execution Of An Agreement Between The City Of Hollywood And Exceletech Coating & Applications, LLC For Construction Services Related To Concrete Repair/Coating Of the Bar Screen Chamber Walls And Ceiling Slabs Under The Bar Screen Chamber Rehabilitation Project At The Southern Regional Wastewater Treatment Plant.
City of Hollywood Staff, As Agreed By The Contractor, Has Executed One (1) Change Order To Date.
Pursuant To Section 38.47(A) Of The Purchasing Ordinance Of The City Of Hollywood, Change Order Means Changes, Due To Unanticipated Conditions Or Developments, Made To An Executed Contract, Which Are Reasonably And Conscientiously Viewed As Being In Fulfillment Of the Original Scope Of The Contract And/Or Matters Directly Related Thereto.
Pursuant To Section 38.47(B)(2)(A) And (B), The City Manager Is Not Authorized To Approve A Change Order Where The Cumulative Effect Of All Approved Change Orders Will Result In The Extension Of A Contract Completion Date By More Than 180 Calendar Days.
Change Order No. 1 Deducted $13,546.64 From The Cost Of The Deleted Work And Granted An 89-Day Time Extension.
During The Concrete Surface Preparation Work, It Was Discovered That The Structural Reinforcement For The Ceiling Slab Lost Its Structural Integrity Since Associated Beams Had Been Severely Corroded Or No Longer Existed.
The City Of Hollywood Decided To Suspend The Bar Screen Chamber Rehabilitation Project Due To The Safety Concern.
City Of Hollywood Staff Included The Design And Replacement Of The Damaged Ceiling Slab Under The Headworks Rehabilitation And Replacement Project.
The City Of Hollywood Reached A No-Cost Agreement With Exceletech Coating & Applications, LLC Whereby The Rehabilitation Work Was Resumed After the Damaged Ceiling Slab was Removed.
An Additional Two Hundred Fifty-Five (255) Days Was Needed To Complete The Design, Bidding, Awarding And Removal Of the Damaged Ceiling Slabs Under The Headworks' Rehabilitation And Replacement Project Approved By The City Commission Via Resolution No. R-2013-233.
Exceletech Coatings & Applications, LLC Completed Its Portion Of the Work on January 21, 2014.
The Administration Recommends That The City Commission Authorize The Appropriate City Officials To Execute The Attached change order No. 2 to the contract between Exceletech Coating & Applications, LLC and the City of Hollywood for a time extension of an additional two hundred fifty-five (255) calendar days for unforeseen site conditions related to the Bar Screen Chamber Rehabilitation Project At the Southern Regional Wastewater Treatment Plant.
If Change Order No. 2 is approved, the cumulative time extension issued for the Bar Screen Chamber Rehabilitation Project At the Southern Regional Wastewater Treatment Plant would be 344 calendar days and the new completion date for the project would be January 21, 2014.
Change Order No. 2 also includes a decrease of $101,231.60, which by itself would not require City Commission approval.
