Explanation:
The Office of Human Resources requires an agreement with a qualified firm to provide Voluntary Vision Insurance to City employees and retirees. Vision Insurance is a supplemental product offered on a voluntary basis without a premium contribution from the City.
The Voluntary Vision Insurance provider shall also provide third party administrative services include performance reports that must include claims experience, capitation or other forms of calculating premium versus benefit.
On January 30, 2015, Request for Proposals (RFP) Number 4448-15-RD was electronically advertised via BidSync in accordance with the City's Purchasing Ordinance, Section 38.42(A).
RFP-4448-15-RD was opened at 3:00 p.m. on February 17, 2015, and resulted in the following five (5) responses:

The RFP document's selection process required an evaluation committee to review and score/rank the responsive proposals based upon the Evaluation Criteria established in the RFP document.
The evaluation of the proposals was based upon the following criteria as established in the RFP: Scope of Services/Administrative Efficiencies, References, Plan Cost, Schedule of Benefits, Location of Support and Assigned Staff.
On February 27, 2015, the proposals were evaluated by a three (3) member committee representing the Office of Human Resources, Planning Department and the Department of Parks, Recreation & Cultural Arts.
The five (5) responsive proposals received were evaluated and the scores were compiled to arrive at the following rankings, (highest score), first choice to fifth choice:

The Office of Human Resources is seeking to engage the services of the highest ranked firm to provide Voluntary Vision Insurance to City employees and retirees and also provide third party administrative services to the City for the program plan.
The fees to be paid for services shall be paid for by the voluntary vision program participants with no cost to the City; and the RFP document provides for an initial term of one (1) year with two (2) additional one (1) year renewal options.
Award is subject to the City's receipt and approval of all insurance certificates required by the City's Risk Manager, and indemnity to the City, along with signed Statement of Hold Harmless and Indemnity in favor of the City.
