Explanation:
The Fire Rescue and Beach Safety Department desires a licensed Physician to provide Medical Director Services.
The Physician will be responsible to provide patient care services on an emergency basis; provide the City's emergency medical services Advanced Life Support (ALS) and the Basic Life Support (BLS) programs; provide instruction of the State of Florida approved training program for certification and renewals for Department Emergency Medical Technicians (EMT's) and paramedics; supervise and accept direct responsibility for the medical performance of the EMT's and paramedics working for the City of Hollywood's Medical Services System; prescribe necessary medical equipment, supplies and drugs; participate in the development of educational programs; review patient care services, and other services as listed in the Physician's proposal.
Notices of Request for Proposal Number RFP-4402-14-JE were mailed on February 3, 2014, and the RFP was electronically advertised on the City's website and Demandstar in accordance with the City's Purchasing Ordinance, Section 38.42(A)(1).
RFP-4402-14-JE was opened at 3:00 P.M., Thursday, February 27, 2014, and resulted in one (1) response as follows:

Evaluation of the proposals was based upon the criteria as established in the RFP as follows: Work Experience; Qualifications; Price; Experience in Teaching, Testing and Evaluating Employees; Emergency Department Physician at Hollywood Facility and Local Preference.
The proposal was evaluated by a committee comprised of the two Division Fire Chiefs and the Human Resources Administrator.
After the proposal was evaluated, it was determined that the proposal received from Dr. Randy S. Katz, D.O. met all of the requirements as listed in the RFP.
The Fire Chief and the Director of Procurement Services recommend that the City Commission authorize the appropriate City Officials to negotiate and execute an Agreement between Dr. Randy S. Katz, D.O. to provide Medical Director Services for the City's Fire Rescue & Beach Safety Department in an amount not to exceed $60,000.00 (annually) for a three (3) year period with the right to renew for two (2) additional two (2) year periods based on satisfactory performance, mutual acceptance and the determination that the all renewals are in the best interest of the City.
Funding for this service is provided in the Medical Services Account Number: 01.2151.00000.522.003116 and shall be requested in subsequent fiscal years.
The City reserves the right to cancel this agreement with or without cause with thirty (30) days notification.
Award is subject to the City receiving all insurance required and approved by the City's Risk Manager and other terms and conditions required by the City Manager and the City Attorney.
