Explanation:

The Department of Human Resources requires an agreement with a qualified vendor  to provide health screening services including a comprehensive medical physical/examination for eligible Fire Rescue & Beach Safety personnel  in order to provide the City’s contractually negotiated annual medical examination benefit paid for by the City.

 

In addition, the Department of Human Resources will make available the health screening services including a comprehensive medical physical/examination to ancillary City personnel with all costs to be borne by the employee and at no cost to the City.

 

The Department of Human Resources staff researched and reviewed medical health screening physical services available and selected Life Extension Clinic, Inc. d/b/a Life Scan Wellness Centers who has offered the required services available on the City of Jacksonville, Florida Contract No. P-41-13.

 

Section 38.40(C)(5) of the Purchasing Ordinance allows the Director to procure, without following formal procedures, all goods and services which are subject of contracts with the state, its political subdivisions, the United States government, other governmental entities, or a corporation not for profit whose members are governmental entities, public officers, or any combination thereof, provided however, the goods and services are: (i) the subject of a price schedule negotiated by the state or the United States government, or (ii) the subject of a contract with another governmental entity or a corporation not for profit whose members are governmental entities, public officers, or any combination thereof, which contract is based strictly on competitive bids or competitive proposals and not on any preference.

 

Health screening services including comprehensive medical physical/examinations for eligible Fire & Rescue Department Personnel/Ancillary Personnel was competitively solicited by the City of Jacksonville, Florida via the issuance, processing and award of RFP #P-41-13.

 

On August 25, 2014, the City of Jacksonville, Florida   awarded RFP #P-41-13 to Life Extension Clinic, Inc. d/b/a Life Scan Wellness Centers located in Tampa, Florida to provide health screening physicals for an initial contract period of six (6) months from October 1, 2014 through March 31, 2015  with the option to renew for three (3) additional one (1) year periods, under the mutually agreed upon terms and conditions and RFP #P-41-13.

 

On May 18, 2015, the City of Jacksonville, Florida renewed the agreement with Life Extension Clinic, Inc. d/b/a Life Scan Wellness Centers to provide health screening physicals for the contract’s initial one (1) year renewal period from April 1, 2015 through March 31, 2016 with the option to renew for two (2) additional one (1) year periods, under the mutually agreed upon terms and conditions and RFP #P-41-13.

                     

The Director of Human Resources and the Director of Procurement Services have recommended that the City Commission approve the execution of the attached blanket purchase order for an initial period expiring on March 31, 2016 with the option to renew for two (2) additional one (1) year periods for health screening services, for an estimated expenditure of $75,000.00.

 

Award is subject to the City’s receipt and approval of all insurance certificates required by the City’s Risk Manager, along with signed statements of Hold Harmless and Indemnity to the City.

 

Funding for this agreement has been provided in the FY16 Health Claims Reserves budget account number 58.1241.00000.590.009199 and will be budgeted in subsequent fiscal years.
